MDS Coordinator
Summary:
Responsible for the timely scheduling, completion, and transmission of MDS.
Qualifications:
Education:
- Graduate of an accredited School of Nursing, RN mandatory.
Licenses/Certification:
- Valid nursing license in the state employed.
- Valid CPR certification.
Experience:
- One year of nursing experience preferred.
Essential Functions:
- Monitors and guides the completion of PPS and OBRA assessments and RAPS.
- UR Coordinator.
- Oversees gathering of information for assessments and care plans.
- MAR’s treatment, flow sheets, physician orders.
- Assesses prospective residents in conjunction with other team members.
- Data warehouse reports and QI reports.
- Schedules and facilitates care plan conferences.
- Participates on committees.
- Inputs or supervises the input of MDS assessments into the Matrix System, the transmission to the state, and obtains receipt of validation.
- Manages Care Planning Process.
- Performs the functions of a nurse when required.
- Performs other tasks as assigned. On-call.
- Conducts job responsibilities in accordance with the standards set out in the Company’s Code of Business Conduct, its policies and procedures, and Corporate Compliance Agreement, applicable federal and state laws, and applicable professional standards.

UNIT MANAGER MONDAY THRU FRIDAY $5,000 SIGN ON BONUS
HIRE ON BONUS OF $5,000.00
SUMMARY: Responsible for managing the operation of a nursing unit in a large facility.
QUALIFICATIONS:
Education:
Graduate of accredited school of nursing, BS preferred.
Licenses/Certification:
Valid RN license in the state employed.
Valid CPR certification.
Experience:
Seven years experience in nursing, two years of which was supervisory in a long term care environment.
ESSENTIAL FUNCTIONS:
§ Performs general management functions such as hiring, disciplining and evaluating employees.
§ Plans and facilitates meetings and committees to address resident care issues for the unit.
§ Manages area of responsibility with the goal of achieving and maintaining the highest quality of care possible.
Participates in developing, implementing and evaluating programs that promote the recruitment, retention, development and continuing education of nursing staff members.
§ Contacts physicians as necessary and ensures physician’s interventions are timely and appropriate.
§ Reviews staffing patterns and census of nursing units, and reassigns personnel when necessary to ensure staffing meets resident needs and budgetary controls.
§ Monitors the clinical operations of the unit and residents’ conditions and ensures that appropriate and quality care is administered.
§ Obtains medications, supplies, and medical records needed to provide safe, efficient, and therapeutic care to residents on a continuing basis.
§ Assists in the orientation of new personnel, monitors their skills, and guides and observes staff that may need assistance with procedures.
§ Participates in facility “Performance Improvement Program.”
§ Performs other tasks as assigned.

Position: Registered Nurse (RN)
Status: Full Time
Shifts Available: Includes weekend rotation, and a holiday rotation.
